Good Faith Estimate

Your right to a Good Faith Estimate

In compliance with the No Surprises Act (NSA) that went into effect January 1, 2022, I am required to notify all healthcare consumers of your federal rights and protections against “surprise billing,” or unanticipated medical bills.

This act requires that I notify you of your federally protected rights to receive a notification when services are rendered by a non-participating provider and provide options to receive care from an in-network provider if one is available. You may choose to use another provider for therapy or neurofeedback services. Additionally, I am required to provide you with a Good Faith Estimate of the cost of services for the duration of treatment. It is difficult to determine the true length of treatment for mental health care. As a strength-based and client-centered practice, I empower my clients to determine their own course of mental health treatment, including, but not limited to: frequency of treatment and length of service.

You are entitled to a Good Faith Estimate:

  • No less than 3 business days before the scheduled service if scheduled within 10 days of service,
  • No less than 24 hours before the service if scheduled within 3 days of service,
  • Prior to committing to a treatment agreement,
  • Upon your request, within 3 business days of request.

You may, at any time and without penalty, request a new Good Faith Estimate.

What is in a Good Faith Estimate?

Your Good Faith Estimate includes:

  • Description of anticipated services
  • Outline of any potential charges
  • Diagnosis and service codes
  • Estimation of treatment time
  • Necessary disclaimers

If you are having trouble reading or understanding your Good Faith Estimate, I will gladly walk you through it so that we are both on the same page.

What if the bill I receive is different from the Good Faith Estimate?

During your treatment with Deborah L. Fenton-Nichols, PC, you may request a new Good Faith Estimate at any time. We may discuss your care and you may choose to extend treatment or increase or decrease the frequency of your therapy and/or neurofeedback sessions. I am also available for additional sessions between scheduled session which will also change the outcome of your billing. Deborah L. Fenton-Nichols, PC. will also be able to provide updated Good Faith Estimates upon your request.

If your billing is more than $400 different from the estimate(s) you have received, you may contact: The State of Colorado at help@cohealthinitiative.org or https://www.cms.gov/nosurprises/Ending-Surprise-Medical-Bills

What if I disagree with my Good Faith Estimate?

Receiving your estimate does not mean that you are committing to receive care from me. You have the right to seek care from another provider for any reason at any time. If you do not agree with or approve of the terms and conditions in your provided estimate, you may decline care without penalty.

Is therapy right for me?

There are many reasons a person decides to go to therapy.

Some find this a necessary tool for psychological struggles like depression, anxiety, grief and loss.

Others are seeking help with alcohol or substance use issues. Then, there are those individuals who need help with life transitions or change, finding greater balance in daily life, or feeling better about who they are.

How can therapy help?

Therapy can help you gain a better understanding of yourself, your strengths and weaknesses, your patterns in relationships, and your unique needs.

Identify your goals and strategies to meet them. Find resolution to long-standing concerns, issues, worries, or things you feel stuck with.

Learn practical ways to cope with stress, anger, depression, or grief. Gain insight into these old patterns and create new ones.

No therapist can offer guarantees; but if you are ready to be open and honest and to put in the work needed in therapy, you can be confident that together we can make progress.

What can therapists offer?

Compassion, respect, and understanding. A fresh perspective on a long-standing issue. New ways to solve problems.

Honest and direct feedback. Education on psychological and emotional processes.

A space to process your personal history, to find support for current day-to-day stress, and to plan for future goals.

Is the first session free?

I offer a free, 20-minute phone consultation to determine if you and I are a good fit. If it’s not a good fit, I will refer you to another therapist that will fit your needs.

During the phone conversation, feel free to ask whatever questions you have.

If you’re relatively confident you’d like to work with me, you will schedule an intake session where we will begin to determine your needs and goals. The intake session is billed at the normal session rate.

What is the first session like?

Most people think for a long time about making that initial appointment and often feel nervous or anxious meeting a therapist for the first time. This is normal.

Just show up and be yourself. My goal is to make you feel comfortable, relaxed, safe, and positive.

The initial session is the intake session. This is where I will be gathering information about you to help me better serve you. We will discuss what you want from therapy and explore how to meet your goals.

Do you accept my insurance?

Yes!

Aetna       Apostrophe      United/Optum        Foster Source      Bright Health      Cigna     Anthem     Lucent Health

For those clients who want to submit for out-of-network reimbursement, I am happy to discuss providing you with the necessary paperwork. As always, check with your carrier for specifics on your coverage.

What forms of payment do you accept?

I accept cash, check, or credit card at the time of service. While a card is required to be on file and charged for the initial session, you are welcome to pay by cash or check thereafter, if you prefer.  Insurance as listed in the above section.

What is your cancellation policy?

Your scheduled session is reserved for you. By the time we are within a few days of your session, my personal and professional schedules have been arranged around your session, leaving me with no opportunities to fill your session time with another paying client.

Consequently, cancellation needs to be made 48 hours prior to your session. After that point, I consider you as having purchased that time even if you end up not being able to attend.

I am happy to make arrangements to do online video-conference if that accommodation would allow you to not miss your session. Some special accommodations may be made in the event of a legitimate emergency.
